He had been its Managing Director from 2008 till 2019. The Zi is the research institution of the Associations of Statutory Health Insurance Physicians and the National Association of Statutory Health Insurance Physicians (KBV) in the legal form of a foundation. The Zi focuses on care research with medical billing data and drug billing data of its sponsors, thereby covering roughly 90% of the population. From 1998 to 2008, Dr. von Stillfried headed the policy department of KBV. He joined KBV after working for a statutory health insurance company and a health insurance association.
